Use the scheduler to request your preferred appointment time. After you submit your request, you’ll receive an email with your intake packet and a link to complete payment.

Please note: Your appointment will be confirmed when both your intake forms and payment are received. Once everything is complete, you’ll get a confirmation email with all the details.
